If you need more Housing Rehabilitation Inventory Cards, please call/email me to ask for more. Because those cards are printed on heavy cardstock, we cannot put those forms on the web for download.

Historic Significance Response Sheet - The forms submitted with the inventory cards, these sheets contain your return address and the address of the project. To use this form: click in the area near “Your Address” at the top of the form. This will open a text box. Type in your return address. We will return the response sheet using a window envelope, so it is important that your return address is in this section of the form. If this area is left blank, the SHPO cannot review your project. Repeat for the bottom portion of the form. Save the document to your local computer. When you need to submit projects for our review, simply fill in the date, indicate whether the project is a rehabilitation or demolition, identify your source of funding, and click on the gray boxes and type in the project address in the appropriate boxes. You can fit two projects on each sheet of paper – we do not need both portions of the form filed out for each project. After it is filled out, print it, cut it in half, attach each half to an inventory card and mail it to our office. If you are unable to open Word documents, use this .pdf version. Just print it off and fill it in by hand. Unfortunately, the inventory cards, because they are printed on card stock, are not available electronically.  If you need more, send me an email and we’ll get those in the mail as soon as possible. [Yes, we realize this is archaic.  We'd love to have on-line submission capability, but we currently lack the funding for that.]

Longer Section 106 Form - This the longer Section 106 form to be used for any ground-disturbing projects and all projects that are NOT housing rehabilitation projects (for example, new housing construction, non-HUD-funded projects, etc,)
